Directions For Completing GEAP Application
If the student is applying for grades K-4, complete the yellow section of the application and submit it to the Office of Academic Enhancement, 125 S. Clark St., 4th floor, Chicago, IL 60603. (For receipt, please include a self-addressed, stamped postcard with the application.) All applicants for grades K-4 are tested. Completed applications must be postmarked no later than December 21, 2007.
If the student is applying for grades 5-8, complete the yellow section of the application and submit it to the student’s counselor or principal. (Parents are strongly encouraged to request a receipt that indicates the time, date and to whom the application was submitted at the home school.) The counselor or principal will complete the gray and white sections of the application and submit it to the Office of Academic Enhancement, 125 S. Clark St., 4th floor, Chicago, IL 60603. Completed applications must be postmarked no later than December 21, 2007.
Bilingual Program: Indicate if the student is currently enrolled in a bilingual program.
IEP/504: Indicate if the student has an IEP or a 504 Plan. If you indicate yes, include a copy of the IEP or 504 Plan. Testing eligibility for students applying for grades 5-8 will be based on the student’s 2006-2007 ISAT national percentile rank in reading and math, or on a different nationally normed standardized test (i.e., ITBS, TERRA NOVA, etc.). In order to be eligible for testing, students with an IEP or 504 Plan applying for grades 5-8 must score at or above the 70th percentile in one subject (reading or math), and at or above the 60th percentile in the other subject (reading or math).
Birth Date: To be eligible for kindergarten, students must be 5 years old by September 1, 2008. To be eligible for 1st grade, students must be 6 years old by September 1, 2008.
City: The city of Chicago is preprinted on the application. You must live in Chicago to attend a GEAP school/program.
Academic Centers: Select up to three Academic Centers, in your order of preference. Testing eligibility for students will be based on the student’s 2006-2007 ISAT national percentile rank in reading and math, or on a different nationally normed standardized test (i.e., ITBS, TERRA NOVA, etc.).In order to be eligible for testing, students must score at or above the 70th percentile in both reading and math. Regional Gifted Centers and Classical Schools: From the 13 schools, select up to a maxiumu7m of 6 choices in your order of preference. Regional Gifted Centers: Testing eligibility for students applying for grades 5-8 will be based on the student’s 2006-2007 ISAT national percentile rank in reading and math, or on a different nationally normed standardized test (i.e., ITBS, TERRA NOVA, etc.). In order to be eligible for testing, students applying for grades 5-8 must score at or above the 90th percentile in both reading and math. Classical Schools: Testing eligibility for students applying for grades 5-8 will be based on the student’s 2006-2007 ISAT national percentile rank in reading and math, or on a different nationally normed standardized test (i.e., ITBS, TERRA NOVA, etc.). In order to be eligible for testing, students applying for grades 5-8 must score at or above the 80th percentile in both reading and math.
Regional Gifted Centers for English Language Learners: Select up to three Regional Gifted Centers for English Language Learners (ELLs), in your order of preference. Testing eligibility for students applying for grades 5-8 will be based on the student’s 2006-2007 reading and math final grades. In order to be eligible for testing, students applying for grades 5-8 must have no lower than a B in reading and math. The student’s primary language must be Spanish, Polish, or Russian:
IB Preparatory: Select up to two International Baccalaureate Preparatory Programs, in your order of preference. Testing eligibility will be based on the student’s 2006-2007 ISAT national percentile rank in reading and math, or on a different nationally normed standardized test (i.e., ITBS, TERRA NOVA, etc.). In order to be eligible for testing, students must score at or above the 90th percentile in one subject (reading or math), and at or above the 80th percentile in the other subject (reading or math). This application does not guarantee acceptance into an academically advanced school. Test results will be shared among the academically advanced elementary schools. Incomplete applications will not be accepted. Information of students applying for grades K-3 will be shared with the Illinois Institute of Technology for the purpose of scheduling test dates.
